Explicit Consent Text for the Processing of Personal Data

Pursuant to the Law No. 6698 on the Protection of Personal Data and the Regulation on the Processing of Personal Health Data and Ensuring Privacy, I consent to all kinds of personal data, sensitive personal data and personal health data given by me verbally, in writing and/or electronically to “ÖZALTIN TARIM İŞLETMELERİ SANAYİ VE TİCARET A.Ş.”, including data that identifies me or serves to identify me, being processed by “ÖZALTIN TARIM İŞLETMELERİ SANAYİ VE TİCARET A.Ş.”; being shared, as required by work and in order to provide and plan health services, with other persons and organisations where sharing is necessary under the relevant legislation; and to the processing of the personal and sensitive personal data detailed below within the scope defined in the said Law and Regulation:

• Identity information: Data clearly belonging to an identified or identifiable natural person, processed partially or fully automatically or non-automatically as part of a data recording system, containing information about the person's identity; documents such as driving licence, identity card and passport containing name and surname, Turkish ID number, nationality, mother's and father's names, place and date of birth and gender, as well as tax number, social security number, credit card information, signature information, vehicle licence plate, etc.,
• Contact information: Information such as telephone number, address, e-mail address, fax number and IP address, clearly belonging to an identified or identifiable natural person and processed partially or fully automatically or non-automatically as part of a data recording system,
• Physical premises security information: Personal data relating to records and documents taken on entering and while staying in the physical premises; camera records, fingerprint records and records taken at the security point,
• Visual records: Photographs and camera records (excluding those within physical premises security information), and data in documents that are copies of documents containing personal data,
• Sensitive personal data: Data listed in Article 6 of the Law on the Protection of Personal Data (e.g. health data including blood type, biometric data, etc.),
• Request and complaint information: Personal data relating to the receipt and evaluation of any request or complaint addressed to “ÖZALTIN TARIM İŞLETMELERİ SANAYİ VE TİCARET A.Ş.”.

I have read the Clarification Text on the Protection and Processing of Personal Data of “ÖZALTIN TARIM İŞLETMELERİ SANAYİ VE TİCARET A.Ş.”, available on the website, provided in printed and electronic media or sent by SMS to the mobile number I declared as my contact number, and I have read my rights.
